You were charged a copay and coinsurance for the same service. Is that wrong?

It depends entirely on one cell in your plan schedule.

Some services are legitimately priced with both, for example a copay per admission followed by coinsurance on the rest of the stay. If the cell spells out that combination, both charges are correct and there is nothing to dispute.

But if the cell prices the service with only one of the two, and you were charged both, that is a double cost share error.

There is a related trap in the same neighborhood that costs far more, and it is a copay's unit. A copay is always per something: per visit, per day, per stay, per admission. Five hundred dollars per stay for a four day admission is $500. The same figure applied per day is $2,000. One preposition, fifteen hundred dollars.
